How we protect your personal health information
London Vision Training Clinic (London Vision Training Centre Inc.) respects your privacy. We collect, use and share personal health information in line with Ontario's Personal Health Information Protection Act, 2004 (PHIPA) and the standards of the College of Optometrists of Ontario.
This page explains what information we collect, why we collect it, who we share it with, and how you can see or correct your records.
We collect only what we need to care for you, and we collect it from you directly wherever possible.
We share your information only as needed and as PHIPA allows:
We do not sell your information or share it for marketing.
You may withhold or withdraw your consent at any time, except where the law allows or requires us to share information without it. Withdrawing consent may affect our ability to coordinate your care or process an insurance claim, and we'll explain any effect before you decide.
For children, we work with a parent or legal guardian, who makes privacy decisions on the child's behalf as permitted by law.
Your records are kept in secure systems. Access is limited to the optometrists and staff who need it for their work, and everyone on our team is bound by confidentiality. We keep records for as long as the College of Optometrists of Ontario requires, then destroy them securely.
If your information is ever lost, stolen or accessed without authorization, we will notify you as required by law.
You have the right to see and get a copy of your health record, and to ask us to correct anything that is inaccurate or incomplete. Please make your request in writing to Dr. Maciej Suwala (contact details below). We will respond within the time set by law, normally 30 days. A reasonable fee may apply for copies, as permitted by law.
Dr. Maciej Suwala is responsible for privacy at our clinic. If you have a question or concern about how we handle your information, please contact him directly and we will work with you to resolve it.
If you are not satisfied with our response, you may contact the Information and Privacy Commissioner of Ontario.
